An out-of-state collection agency has called my employer informing them if they do not hear back from me within two days, they are going to request from my county Superior Court that an arrest be made at my place of work. Is this legitimate?

That sounds like a violation of the Fair Debt Collection Practices Act. I'd suggest you call an Arizona consumer rights attorney, to see if this agency is worth suing to protect your rights.
