I am 22 years old, my father sexually abused me when I was 12. I want to see him, and his parole conditions forbid that. How can I get that condition changed so our family is whole again.

You need to see a psychologist and get some help with this. After that, after you have gotten some serious and strong help with the damage he did, then you can think about trying to see him.
I have no idea how you can ask the Judge to let this horrible man have the opportunity to see you again. He also needs to seriously change himself.
Please get help. At age 22, you have many years ahead of you.
